How to get your Kentucky medical cannabis card
1. You must have one of the following qualifying conditions
to be eligible for a medical cannabis card.
*Any type or form of cancer *Chronic or severe pain
*Epilepsy or other intractable seizure disorder
*Multiple sclerosis, muscle spasms, or spasticity
*Chronic nausea or cyclical vomiting syndrome
*Post-traumatic stress disorder
2. You must be a Kentucky resident with a valid Kentucky ID.
3. You can not be convicted of a disqualifying felony offense.
Prior to applying to the Kentucky medical cannabis program, you must obtain a medical cannabis certificate from a board certified provider. This certificate confirms that the patient meets program criteria and has a qualifying medical condition that medical cannabis is recommended as part of the patient's treatment plan.
Schedule your in person appointment with Danielle Guminski, a licensed medical cannabis provider. Danielle will review your medical history, educate you on the benefits and risks of medical cannabis, and provide a written and electronic medical cannabis certificate.
Danielle will then walk you through the Kentucky cannabis program application.
