This certificate confirms that the patient meets program criteria and has a qualifying medical condition that medical cannabis is recommended as part of the patient's treatment plan.
To start the process you need to schedule your initial in person visit with Danielle Guminski, a licensed medical cannabis provider. Danielle will review your medical history, educate you on the benefits and risks of medical cannabis, and provide a written and electronic medical cannabis certificate.
Danielle will then help you complete your application to the Kentucky cannabis program!
